Forms
Each player is required to have the following forms on file prior to tryouts (current for the 2024-2025 school year). A doctor visit is required to complete the GHSA Physical form. Please do not email your physical or other forms to coaches to print out for you. These forms should be submitted to Dragonfly. All players must register for an account at:
